I give my child permission to participate in a non-school sponsored activity during the 2024-2025 school year. I expressly understand and acknowledge that my child is under the exclusive care and supervision of third-party vendors during all extra-curricular activities and that St. Brendan School bears no duty or responsibility for those activities.

I assume all risks and hazards incidental to such participation and indemnify and agree to hold harmless St. Brendan School, including but not limited to its staff, coaches, and teachers (“Indemnitees”) whether or not any resulting injuries or damage shall be caused during my child’s participation in extra-curricular activities.
